APPOINTMENTS

We strive to meet our patient's needs with timely, convenient appointment hours. We do reserve appointment times for acutely ill patients, and advise you to schedule appointments for routine visits and physicals well in advance. We try to have appointments ready to schedule for at least three months in advance.

CoPayments may be part of your contractual agreement with your health insurance carrier. Copays are due at each appointment and are the patients' responsibility. In the event that we must bill for a copay that is not received at the time of your visit, effective July 1, 2006, an administrative fee of $25.00 will be added to the charge for the copayment.

We request at least 24 hours notice to cancel an appointment. We do realize emergencies occur, and ask you contact our office as soon as possible so we may document the emergency. Missed appointments are appointments not kept or not cancelled one hour prior to the appointment time by the patient or guardian. We do notify patients of missed appointments through letters and charge an administrative fee of $25.00.
