Forms and Letters
Forms to be filled out by this office for any reason may require up to one week for the physician's nurse to complete. This includes disability forms and record requests. Please plan accordingly. We request forms be brought in during your appointment for completion. If you are not able to bring your form it is still ok. We do charge $30.00 to complete forms. We also have a release that you would need to fill out and sign prior to the office filling out your forms.
Letters requested from a physician on behalf of a patient may be charged at the doctor's discretion. A minimum charge of $10 should be expected for letters because of extra time taken by the physician and typist as well as stationary costs. If the patient just needs a work or school excuse that will be written after the appointment, and there will be no chargef or this.
